APPLICATION REQUIREMENTS

Category 1: FILIPINO UNDERGRADUATE STUDENTS FROM LOCAL SCHOOLS

  1. National Statistics Office (NSO) certified true copy of birth certificate. Photocopy is acceptable provided true copy can be shown for verification, when requested.
  2. Certificate of naturalization for naturalized Filipinos.
  3. Certified true copy of transcript of records (NOTE: For graduating students, grades for your final year may not have been encoded yet at the time of application. This is acceptable. However, please note that if you should get a failing grade on your last year, you may forfeit your slot.)  (NOTE: Applicants from UST who have not taken any subject/s outside UST need NOT submit a copy of their transcripts of records.)
  4. Photocopy of parent’s/parents’ and/or sibling’s/siblings’ diploma/s if UST graduate/s.
  5. Photocopy of NMAT scores. Please check the list of NMAT dates that we will considered for the present application process.
  6. Two (2) certificates of good moral character (cf. Admission Policies #6)
  7. Accomplished printed copy of the Online Application Form
  8. Accomplished printed copy of the Health Questionnaire Form duly signed by a licensed Physician (2 pages printed back-to-back)
  9. Photocopy of Baptismal certificate (if applicable)
  10. Photocopy of certificates of extracurricular activities (leadership/civic/volunteer work)
  11. Photocopy of official receipt or bank deposit slip/payment slip of the processing fee of Three Thousand Pesos (PhP 3,000)

Category 2: FILIPINO GRADUATES FROM LOCAL SCHOOLS

  1. Same as for Category 1 except for item 3
  2. Transcript of records of a completed four-year BA or BS course      
  3. In addition, the following is required if applicable:
    1. Professional Licensure certificate or post-graduate (masters/doctorate) diploma.
      Photocopy is acceptable provided true copy can be shown for verification, when requested.

Category 3: FILIPINO UNDERGRADUATE STUDENTS FROM FOREIGN SCHOOLS

  1. Same as for Category 1 except for item 11
  2. Photocopy of official receipt or bank deposit slip/payment slip of the processing fee of Three Hundred Twenty U.S. dollars (USD 320 or PhP 17,600)
  3. In addition, the following is required:
    1. TOEFL/ESL results for Non-English speaking international students
      * Note Admission Policies #5 regarding the language requirement for English and Filipino (Tagalog)

Category 4: FILIPINO GRADUATES FROM FOREIGN SCHOOLS

  1. Same as for Category 1 except for item 3 and 11
  2. Transcript of records of a completed four-year BA or BS course*
  3. Photocopy of official receipt or bank deposit slip/payment slip of the processing fee of Three Hundred Twenty U.S. dollars (USD 320 or PhP 17,600)
  4. In addition, the following are required if applicable:
    1. Professional Licensure certificate or post-graduate (masters/doctorate) diploma.

Category 5: RESIDENT ALIENS

  1. Same as for Category 1 except for items 1, 2, and 11
    1. If the applicant is a graduate (as opposed to an undergraduate), then item 3 will have to be modified such that the transcript of records should be of a completed four-year BA or BS course. In addition, Licensure certificates and/or post-graduate (masters/doctorate) diplomas will have to be submitted. Photocopies are acceptable provided that the original documents can be shown for verification, when requested.
  2. Certified true copy of birth certificate (notarized copy is acceptable)
  3. Certificate of citizenship
  4. Photocopy of official receipt or bank deposit slip/payment slip of the processing fee of Three Hundred Twenty U.S. dollars (USD 320 or PhP 17,600)
  5. In addition, the following are required if applicable:
    1. Scanned copy of ACR I-Card
    2. Notarized Affidavit of Support
    3. TOEFL/ESL results for Non-English speaking international students

Category 6: NON-RESIDENT ALIENS

  1. Online Application Form
  2. Certified true copy of birth certificate (notarized copy is acceptable)
  3. Certificate of citizenship
  4. Transcript of records* of a completed four-year BA or BS course.
  5. Photocopy of parent’s/parents’ diploma/s if UST graduate/s
  6. Photocopy of NMAT scores. Please check the list of NMAT dates that we will consider for the present application process.
  7. Two certificates of good moral character (cf. Admission Policies #6)
  8. Notarized affidavit of support* with bank statement
  9. Two (2) original copies of Personal History Forms
  10. Police clearance* from place of residence
  11. Accomplished printed copy of the Online Application Form
  12. Accomplished printed copy of the Health Questionnaire Form duly signed by a licensed Physician (2 pages printed back-to-back)
  13. Photocopy of official receipt or bank deposit slip/payment slip of the processing fee of Three Hundred Twenty U.S. dollars (USD 320 or PhP 17,600)

* Transcript of records, notarized affidavit of support, and police clearance must be authenticated by the Philippine Consulate Office or Embassy - they must bear the red ribbon of the Consulate.

** Accepted applicants belonging to Categories 3, 4, 5, and 6 are required to pay an out-of-state fee of US$15,000.00 upon enrolment. The out-of-state fee is a one-time payment on enrolment and is not refundable.

The following SCREENING CRITERIA are used in the overall evaluation and selection of applicants for admission into the Medical School:

  1. Scholastic standing
  2. NMAT (National Medical Admissions Test)
  3. Entrance examination, if administered
  4. Loyalty (e.g., if parents are alumni, if applicant is a UST graduate, etc.)
  5. Extracurricular activities (religious, leadership, civic, volunteer work)
  6. Professional licensure certificates and other graduate/post-graduate diplomas
